Unfortunately, many times patients create their own barriers that hamper improvement. For this discussion:

Think about the clients you see in your own nursing practice and describe at least two barriers that hinder a plan of care that includes health promotion and wellness.
Explain how each barrier works to inhibit the concepts of health promotion and wellness, using information found in this week’s readings as well as other scholarly sources.
