Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?
Medications:
IV furosemide (Lasix): It reduces extravascular volume by inducing diuresis hence decrease preload and the work of the heart in pumping blood hence prevent progression of heart failure. That is Lasix is a diuretic that releases excess fluid, thus reducing strain on the circulatory system (Howard, 2001).
Enalapril (Vasotec): This medication is an antihypertensive which causes vasodilation of peripheral vessels decreasing afterload resistance hence effective in heart failure. Its help to lower blood pressure, increase cardiac output and exercise tolerance without changing the heart rate (Gierrthmuehlen, 2016).
Metoprolol (Lopressor-it’s a beta blocker and usually used to treat high blood pressure, chest pain, and heart stress after a heart attack due to its antagonistic effect on of beta receptors on the heart. It slows the heart rate, allowing the left ventricle to fill more completely. The slowing of the heart (or the conduction through the AV nodes) helps to regulate A fib. A slower heart rate will assist the pt to feel less anxious.
IV morphine sulfate (Morphine) is a narcotic analgesic use to manage pain. It also increases O2 supply to the heart while decreasing the heart’s O2 demand.

High blood pressure- Uncontrolled BP is a high-risk factor for developing CHF. When pressure is blood vessels is too high the heart has to pump harder than normal to keep up, and over time it causes the heart to get weaker.
