Purpose of this assignment

The health and social act 2012 requires health practitioners and social care managers to be more innovate, more productive and more accountable while delivering the NHS, Adult Social care and Public health outcomes (quality, safety and patient experience)

The aim of this unit is to develop understanding of the values, theories and policies underpinning health and social care practice and the mechanism that exist to promote good practice.
PLEASE NOTE: THIS ASSIGNMENT WILL BE IN THREE PARTS.

Before addressing the 3 parts below, you could consider to develop your own case study (for D1, D2) based on:

a) Other people’s/ colleagues’ work experiences

b) Your own work experience

c) A collaborative case study, i.e. a collection of various people/ colleagues work experiences.

Part 1. An Essay (500-1000 words) based on the case scenario provided below (1.1 , 1.2, 1.3 and 1.4).

Part 2. PowerPoint Presentation (12-15 slides) with reference to the case scenario (2.1, 2.2 , 2.3 and 3.1, 3.2 and 3.3, i.e. 2 slides for each assessment criteria)

Part 3. Reflective account (500-700 words plus an optional 5 min audio or video recording) based on own workplace experience within Health and social care sector only. (4.1, 4.2 and 4.3)

PLEASE use the Scenario below for Part 1 (Essay) and Part 2 (PowerPoint Presentation slides) Only. Other case studies will be introduced as well as you will have the chance to develop and use your own case study.

East & West Care Home is situated in London and caters to care needs of vulnerable people. The service users come from diverse ethnic and religious background and have varied physical and medical needs. (You may choose the category of service users you work with).

You have recently been appointed as Manager in East & West Care Home. During the first formal meeting with the Board of Directors you were informed about the various problems at Waltham care home and the recent visit of the CQC. There are various complains from the residents and families about the quality of care, staff errors due to insufficient knowledge or application of clinical-care standards and protocols, lack of guideline and inadequate supervision that compromised the safety and efficiency of care.

There are allegations of poor care and mistreatment of the elderly residents for example underfeeding of the residents, left soiled in their beds and unexplained bruising. There have also been some cases of stealing cash from the residents and a case worker was suspended over allegations that he had mistreated the residents. There had been some cases where residents were given wrong doses of medication, and in one case staff falsified medical records after realising a patient had been overdosed and shredded the evidence.

CQC recently published the findings following an inspection of services provided East & West Care Home Ltd and the report was discussed in Board meeting.

CQC report states: After considering a range of evidence inspectors conclude that the registered provider, East & West Care Home Ltd, had failed to ensure that people living in the Care Home were adequately protected from risk, including the risks of unsafe practices by its own staff.

The report concludes that there was a systemic failure to protect people or to investigate allegations of abuse. The provider had failed in its legal duty to notify the Care Quality Commission of serious incidents including injuries to patients or occasions when they had gone missing.

Inspectors said that staff did not appear to understand the needs of the people in their care, adults with learning disabilities, complex needs and challenging behaviour. People who had no background in care services had been recruited, references were not always checked and staff were not trained or supervised properly. Some staff were too ready to use methods of restraint without considering alternatives.

Inspectors who visited East & West Care Home considered taking urgent action to close the center, but decided that it was in the best interests of the patients to allow NHS and local authority commissioners further time to find alternative placements. CQC ensured that there would be an immediate stop on admissions and that extra staff would be brought in to protect patients until they could be moved.

The report found that East & West Care Home Ltd was not compliant with 10 of the essential standards which the law requires providers must meet. CQC’s findings can be found below.

● The managers did not ensure that major incidents were reported to the Care Quality Commission as required.

● Planning and delivery of care did not meet people’s individual needs.

● They did not have robust systems to assess and monitor the quality of services.

● They did not identify, and manage, risks relating to the health, welfare and safety of patients.

● They had not responded to or considered complaints and views of people about the service.

● Investigations into the conduct of staff were not robust and had not safeguarded people.

● They did not take reasonable steps to identify the possibility of abuse and prevent it before it occurred.

● They did not respond appropriately to allegations of abuse.

● They did not have arrangements in place to protect the people against unlawful or excessive use of restraint.

● They did not operate effective recruitment procedures or take appropriate steps in relation to persons who were not fit to work in care settings.

● They failed in their responsibilities to provide appropriate training and supervision to staff.

From this point, you will then also need to more specifically explain how local policies and procedures (in your local health area, i.e. local hospital etc) have been developed in accordance with national and policy requirements.

Finally, please evaluate the impact of policy, legislation, regulation, and codes of practice on organisational policy and practice of your local hospital or health organisation.
Part 2 continuation (3.1, 3.2 and 3.3 and also opportunities to meet M1, M2, M3, D1)

In your PowerPoint presentation, you have to firstly explain the theories that underpin health and social care practice. You then need to analyse how social processes impact on users of health and social care services.

Finally, please reflect either on your practice or on your experience as a user of services from your local hospital and evaluate the effectiveness of inter-professional working.
Part 3 Reflective account (500-700 words plus an optional 5 min audio or video recording) based on own workplace experience within Health and social care sector only. No reference to the case scenario. (4.1, 4.2, 4.3. and also opportunities to meet M1, M2, M3, D2, D3).

In this part of your assignment, you will need to present a reflective report on the amassment criteria based on your own work experience within health and social care sector. In order to achieve D2 and D3, you will also need to submit either an audio or a video recording of you presenting your slides.

In your REPORT, you should explain own role, responsibilities, accountabilities and duties in the context of working with those within and outside the health and social care workplace. You will also need to evaluate own contribution to the development and implementation of health and social care organisational policy. Finally, you will need to make clear recommendations to develop own contributions to meeting good practice requirements.
